BJC Home Care is searching for a Registered Nurse to serve as a Palliative Care Specialist at Memorial Hospital Alton. Do you have hospice, palliative or oncology experience? If so, this may be your next adventure!

Schedule

Monday - Friday

8:00 am - 4:30 pm, with some flexibility

No weekends/holidays

2-5 years of nursing related experience required. Palliative care experience preferred. 

Overview

BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

The BJC Home Care Services Supportive Care staff offer patients with life-limiting illness an opportunity to ease the transition from curative treatment to Hospice care through a unique program. Supportive Care offers a bridge between curative and terminal care. It is a multidisciplinary approach, holistic in nature, centered on the needs of the patient and patient's family, that provides coordinated medical, emotional and spiritual care.

Preferred Qualifications

Role Purpose

Responsible for assessing and coordinating the physical, psychosocial and spiritual needs of the patient and family at end-of-life, as well as pain and symptom management at all stages of the chronic disease process. Develops and coordinates the palliative care plan with physicians and nurses, along with services such as Care Coordination, Pastoral Care, Home Health and Hospice care.

Responsibilities

Consults with physicians, patients, families and the Palliative Care Team to develop and modify individualized plans of care.

Provides ongoing case management for seriously ill and end of life patients.

Provides educational opportunities for physicians, nurses, and other allied health care providers regarding palliative care and advanced directives, and integrates palliative care topics into community education programs.
